Please have both parents sign the form, enclose a $15 check made out to C.H.I.E.F., and mail to: Jason Baity, 12935 School Creek Rd. St. George KS 66535

DEADLINE: Please mail your application form by AUGUST 15th proceeding the school year.

Information from applications received after this deadline will not be included in the directory.
